Job summary

GBMC HealthCare in Baltimore seeks a Hospice Admissions Nurse to assess patients at home or in hospitals and to prepare entry into Gilchrist Hospice Care services; the nurse also serves as an ambassador for GBMC HealthCare within the service area. Education requires a Diploma, ADN and/or BSN with a current MD nursing license.

Candidates should have three years of nursing experience; home health or hospice experience is preferred.

Qualifications

• Diploma, ADN and/or BSN from an accredited institution.
• Current MD nursing license.
• Three years of nursing experience; home health or hospice preferred.

Responsibilities

• Completes initial assessment of patient and family/caregivers to ascertain Hospice eligibility and needs.
• Prepares and implements the initial care plan for patient/family unit with Medical Director approval.
• Provides direct nursing care to patients to maintain highest level of care.
• Teaches family members or hired caregivers patient care skills including pain control and medications.
• Ensures arrangements for home equipment are made available.
• Participates in Admissions department projects and process improvement strategies.
