Job summary

Qualifications

• Diploma, ADN and/or BSN from an accredited institution.
• Current MD nursing license.
• Three years of nursing experience; Home Health or Hospice experience preferred but not required.

Responsibilities

• Completes initial assessment of patient and family/caregivers to determine hospice eligibility and needs.
• Prepares and implements the initial care plan for patient/family with Medical Director approval.
• Provides direct nursing care to patients to maintain highest level of care.
• Documents clinical findings to assure initial hospice eligibility and participate in discharge planning with relevant care teams.
• Teaches family members or hired caregivers patient care skills (pain control, injections, IV care, vital signs) and ensures equipment at home is arranged.
