Job summary

Gilchrist Hospice Care is seeking a Hospice Admissions Nurse to assess patients in their homes or hospitals and prepare them for entry into our spectrum of services. The role acts as an ambassador for Gilchrist Hospice Care within the service area.

The nurse reports to the Admissions Team Manager and collaborates with the Medical Director to develop initial care plans, ensuring the highest level of patient care and smooth transitions between services.

Qualifications

• RN degree and license.
• Three years nursing experience.
• Hospice/home health experience preferred.
• Knowledge of infection control and patient safety.
• Ability to educate patients and caregivers.

Responsibilities

• Complete initial assessment of patient and family to ascertain hospice eligibility.
• Prepare initial care plan with Medical Director approval.
• Provide direct nursing care to maintain highest level of care.
• Inform team leader of referrals and use of community resources.
• Document findings to support eligibility and participate in discharge planning.
• Teach family/caregivers patient care skills and administer medications.
• Arrange home equipment and supplies as needed.
• Attend to patient/family at time of death if applicable.
• Participate in admissions projects and process improvements.
• Maintain relationships with referral sources and healthcare administrators.
