Gentiva Hospice is seeking an experienced Hospice Admissions Nurse to be the face of our hospice services. You will assess eligibility, clearly explain options, and guide families through admissions with empathy and clinical confidence.

You will visit patients in homes, hospitals, or facilities, coordinate with physicians and care teams, and complete documentation in compliance with state and federal rules. Monday–Thursday, 10am–9pm in Texas.

Qualificações

• 1+ year of hospice nursing experience.
• Active RN license in the state of employment (or eligible to obtain).
• Ability to work Monday through Thursday 10am–9pm.
• A hospice heart: compassionate, empathetic, and patient-centered.
• Strong assessment, communication, and interpersonal skills.
• Comfortable visiting patients in hospitals, SNFs, or homes.
• Valid driver’s license, auto insurance, and reliable transportation.
• Current CPR certification.

Responsabilidades

• Visit patients and families in homes, hospitals, or facilities to assess eligibility for hospice care.
• Explain hospice philosophy and services clearly, with compassion and clinical confidence.
• Guide patients and families through the admissions process for a smooth start to care.
• Coordinate transitions of care with referring physicians, case managers, and staff.
• Use the nursing process to assess needs, support planning, and document findings.
• Build trust and provide emotional reassurance during decision making.
• Complete required documentation and ensure regulatory compliance.
• Collaborate with the interdisciplinary team to ensure timely admissions and handoffs.
