Transitions Hospice – Indiana seeks a Care Coordinator to expand referrals and build trust within the Lake, Porter, Jasper, and surrounding counties. You will manage relationships with physicians, nursing homes, and hospitals, while delivering education to accounts about hospice care.
You’ll lead territory development, collaborate with the care team, and support admissions with on-call rotation. Travel within the service area is required, and a strong driver’s license is essential.
