Luminos Hospice is seeking an On-Call RN to provide nursing care for patients in their homes during off-hours and weekends. You will coordinate with the Interdisciplinary Team and families to ensure quality end-of-life care.

Requirements include an active State RN license, CPR certification, and 2 years in clinical care with 1 year of hospice experience. Valid driver’s license and reliable transportation preferred.

Qualifications

• Current State RN license in the state of practice.
• Current CPR Certification.
• Two (2) years' experience in a clinical care setting and one (1) year of full-time hospice experience.

Responsabilités

Job overview:

Luminos Hospice is seeking a skilled and compassionate On-Call RN to join our team. As an On-Call RN, you will be responsible for providing quality care to our patients during off-hours and weekends. Primary functions are to administer nursing care for patients of all ages in their place of residence and to coordinate care with the Interdisciplinary Group/Team, patients, families, and referring agency. Assumes the responsibility for, and participates in, the coordination of care.

Requirements

Licensure:
