Job summary

Luminos Hospice in Missouri is seeking an RN Case Manager to lead a care team and coordinate patients' home-based hospice services.

The role emphasizes collaboration with the clinical director, education for families, timely documentation, and ensuring compliance with state and federal regulations.

Qualifications

Responsibilities

• Coordinates and oversees all aspects of patients' home care.
• Collaborates with patient care teams to develop and implement care plans.
• Educates families on patient condition, needs and care.
• Performs head to toe physical assessments.
• Checks vital signs and manages medications.
• Administers and maintains catheters.
• Complete documentation timely and accurately.
• Supervises HHAs.
• Regularly communicates patient progress to the clinical manager, physician, and care team.
• Other duties as assigned.

Job Summary:

As an RN Case Manager of Luminos Hospice you will work with the Clinical Director to lead a care team including LPN(s), LVN(s), Aides, MSW and Chaplain to provide best in class hospice care.

Responsibilities:

• Coordinates and oversees all aspects of patients' home care.
• Collaborates with patient care teams to develop and implement care plans.
• Educates families on patient condition, needs and care.
• Performs head to toe physical assessments.
• Checks vital signs and manages medications.
• Administers and maintains catheters.
• Complete documentation timely and accurately.
• Supervises HHAs.
• Regularly communicates patient progress to the clinical manager, physician, and care team.
• Other duties as assigned.

Requirements

Licensure: Current State license as a Registered Nurse.

Current driver's license.

Current CPR Certification.

Experience: Two (2) years' experience as a Registered Nurse in a clinical care setting and one (1) year full-time hospice preferred.

Skills: Nursing skills as defined as generally accepted standards of practice.

Knowledge of state and federal regulations.

Ability to work independently and make sound decisions.

Strong attention to detail and organizational habits.

Excellent verbal and written communication skills.

Good interpersonal skills.

Transportation: Reliable transportation and valid auto liability insurance.
