Job summary

Jobtailor is seeking a Service Delivery Coordinator in Minnesota to develop and manage CFSS-based service plans for participants. You will engage with participants, DHS, and service providers to ensure plans meet needs and comply with current policies.

The role requires a degree in a related field or equivalent experience in direct services, plus familiarity with HCBS/CFSS. Strong communication and organizational skills are essential as you coordinate budgets and documentation.

Qualifications

• Bachelor's degree in Human Services, Social Work, Psychology, Healthcare, Rehabilitation, or a related field OR One year of full-time experience providing direct services to individuals with disabilities or older adults OR Personal experience coordinating your own long-term support services
• Preferred Experience in case management, care coordination, social services, disability services, healthcare navigation, or aging services.
• Knowledge of Minnesota HCBS, PCA, CFSS, waiver programs, or related services.
• Experience managing budgets, service plans, or participant-centered programs.
• Proficiency with Microsoft Office and electronic documentation systems.
