Qualifications

• RN with a bachelor’s degree in nursing (preferred).
• 1+ year of clinical RN experience; homecare exposure bonus.
• Strong verbal and written communication for patient/family education.
• Proficient with EMR and digital tools to enhance care.
• Reliable car and valid driver's license for patient-home visits.

Responsibilities

• Complete two-week orientation and learn procedures and EMR system.
• Assess patients with complex health needs and develop personalized care plans.
• Educate patients and families to enable better health outcomes.
• Collaborate with VNA clinicians and external providers for seamless care.
• Document patient changes and update care plans accordingly.
