Job summary

Central Vermont Home Health & Hospice is seeking a Homecare Registered Nurse Resident in Vermont. This trainee role leads to case management responsibilities and is designed for new nursing graduates or nurses with up to two years of experience.

A valid VT license and driver's license are required, with a plan to achieve full proficiency within six months. You will work within a multidisciplinary team, providing goal-oriented home health care, collaborating with physicians, and coordinating

Qualifications

• Graduate of an accredited school of nursing with an associate or bachelor's degree in nursing.
• Vermont nursing license valid by date of employment.
• Valid Vermont driver's license.

Responsibilities

• Develops, communicates, and coordinates a goal-oriented plan of care for each patient in collaboration with the care team and family.
• Implements plans of care and provides nursing interventions and patient/family teaching.
• Maintains up-to-date electronic documentation of care provided.
• Meets nursing visit performance standards including OASIS accuracy and productivity.
• Participates in primary on-call, weekend, and holiday on-call as needed.
• Supervises LPN and LNA staff as required by state/federal guidelines.
