Central Vermont Home Health & Hospice is seeking a Homecare Nurse Resident to begin as a case manager leading care plans. The position is designed for new nursing graduates, or nurses with up to 2 years’ experience, with a six‑month training period.
You will work with an interdisciplinary team to coordinate care, perform on-call duties, supervise LPN/LNA staff as required, and help ensure high-quality home health services to our community.

We are seeking for an individualized goal-oriented Homecare Nurse Resident. This position serves as Case Manager in cases in which this position is the lead discipline. This position is designed for the newnursing graduate, the nurse with preferably2 years' experience, or a nurse returning to the workforce. The expectation is to achieve ability to be proficient in the essential functions of this position within 6 months of being hired in the trainee position.
