Job summary

Central Vermont Home Health & Hospice is seeking a Homecare Nurse Resident to begin as a case manager leading care plans. The position is designed for new nursing graduates, or nurses with up to 2 years’ experience, with a six‑month training period.

You will work with an interdisciplinary team to coordinate care, perform on-call duties, supervise LPN/LNA staff as required, and help ensure high-quality home health services to our community.

Qualifications

• Graduate of an accredited school of nursing with an associate or bachelor's degree in nursing.
• Vermont nursing licensure active by start date.
• Valid Vermont driver’s license.
• Ability to pass background and caregiver checks.

Responsibilities

• Develops and coordinates a goal-oriented plan of care with interdisciplinary team.
• Implements plans of care and provides nursing interventions.
• Maintains up-to-date electronic documentation of care.
• Meets nursing visit performance standards (OASIS accuracy, productivity).
• Participates in on-call, weekends, and holidays as needed.
• Supervises LPN and LNA staff as mandated.
