Job summary

Helen Ross McNabb Center seeks a Healthy Families Family Support Specialist to provide home visiting and family-centered case management. You will work with families in their homes, ensuring well-child visits and medical homes are established, with a focus on evidence-based practices and family resilience.

The role requires excellent communication, trauma-informed care, and the ability to maintain caseload standards while coordinating referrals and community resources.

Qualifications

• Bachelor's degree in Social Work, Psychology, or related field preferred.
• High school diploma with lived experience required if not Bachelor's Qualified.
• AIMH Infant Mental Health Endorsement preferred; required within first year if not endorsed.

Responsibilities

• Initiates and maintains regular long-term contact with families for five years with each family.
• Ensures services are provided in the family's home and visits last about one hour.
• Maintains caseload per Healthy Families America productivity standards.
• Provides direct family-centered case management and ensures medical homes and well-child exams are maintained.
