Job summary
Signify Health, part of CVS Health®, seeks a Full Time Physician to provide in-home health evaluations. The clinician will visit members’ homes, review medical history, perform physical assessments, and coordinate with health plans for diagnostic services.
The role involves travel within licensed states, virtual visits as needed, and a comprehensive benefits package for full-time employees.
Qualifications
- Active, unrestricted licenses in coverage area(s).
- Multi-state licenses preferred.
Responsibilities
- Conduct in-home health evaluations (~45 minutes) to assess health status.
- Perform medication and medical history review and physical evaluation.
- Provide services as indicated by health plan including diagnostic tests when ordered.
