Job summary

Optum is seeking a Nurse Practitioner or Physician Assistant to perform in-home per-assessment clinical evaluations for the HouseCalls team in Apache County, AZ. The role emphasizes preventive home care, coordination with PCPs, and improving patient well-being through comprehensive assessments.

Flexible schedules with 5–10 assessments per week, no on-call, and a $5,000 sign-on bonus for external candidates are offered. Travel within Arizona is required as needed to support patient care goals.

Qualifications

• Active NP or PA license in good standing and able to obtain by start date.
• Active national NP or PA certification or ability to obtain by start date.
• Ability to perform physical requirements including lifting a 30-pound bag, stairs, and varying dwelling conditions.
• Reliable transportation to complete home visit assessments.

Responsibilities

• Conduct in-home assessments including medical history, systems review, medication reconciliation, vitals and full physical exam.
• Perform evidence-based screenings including point-of-care testing when appropriate.
• Identify diagnoses to be used in care management and active medical management.
• Formulate a list of current and past medical conditions based on clinical findings.
• Communicate assessment findings to inform the member's PCP of potential gaps in care.
• Identify urgent and emergent situations and intervene appropriately.
• Educate members on disease processes, medications and compliance.
