Job summary

Optum is seeking a Nurse Practitioner or Physician Assistant, Per Assessment, to join our Home and Community Care team in Apache County, Arizona. In this role you will perform in-home annual assessments, coordinate follow-up care, and support preventive health for vulnerable patients.

Flexible scheduling with 5–10 assessments per week, no on-call or weekends, and a $5,000 sign-on bonus for external candidates. Remote travel up to Arizona statewide may be required with competitive compensation.

Qualifications

• Active NP/PA license or ability to obtain by start date.
• National NP/PA certification or ability to obtain by start date.
• Ability to perform physical exams and home-visit assessments.

Responsibilities

• Conduct in-home assessments including medical history, vitals, review of systems, and physical exam.
• Perform evidence-based screenings and point-of-care testing as appropriate.
• Identify diagnoses for care management and active medical management.
• Communicate assessment findings to inform the member's PCP of care gaps.
• Educate members on disease processes, medications, and adherence.
