Job summary

Hebrew SeniorLife Hospice Care, Inc. is seeking an Occupational Therapist to implement care standards and collaborate on patient plans in a home-health setting in Massachusetts.

The role involves comprehensive assessments, home safety evaluations, and ongoing plan development with the care team. Strong communication, independence, and EMR proficiency are required. On-site with remote support as applicable.

Qualifications

• Licensed Occupational Therapist in Massachusetts.
• Graduation from ACOTE/AMA-AOTA-accredited OT program.
• CPR certification maintained.
• Valid driver's license and reliable transportation.
• Strong oral and written communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Experience with EMR systems and clinical documentation.

Responsibilities

Job description

Position Summary:

The occupational therapist is responsible for the implementation of standards of care for occupational therapy services and for adherence to the plan of care established by the physician.

Position Responsibilities Include:

• Makes home visits to patients in designated geographic territories.
• Performs thorough evaluations of patients’ functional status, including muscle function, endurance, visual coordination, written and verbal communication skills and ADL and IADL performance.
• Evaluates the home environment for hazards or barriers to more independent living and identifies equipment needs.
• Continually assess and revise the occupational therapy care plan, and participate with other team members in the multidisciplinary care plan as appropriate.
• Develops treatment program and establishes goals for improved function.
• Communicates plan of care to PCP or referring physician.
• Teach new skills or retrain patient in once familiar daily activities that have been lost due to illness or injury.
• Train patient in the use of prosthetic and/or orthotic devices.
• Maintains appropriate clinical records, clinical notes, and reports to the physician/appropriate team member any changes in the patient’s condition.
• Complete OASIS documentation with accuracy and submits to the office within 24-48 hours on a consistent basis.
• Maintains contact/communication with other disciplines involved in the patient’s care to promote coordinated efficient care.
• Meets with rehabilitation manager on a regular basis to facilitate patient’s coordinator of care.
• Attends and contributes to in-services, case conferences, and team meetings as required by the agency to ensure coordinated and comprehensive plans of care for the patients.
• Identifies patient and family/caregiver needs for other home health services and refers as appropriate.
• Instructs patient’s family/caregiver and other organization health care personnel in patient’s treatment regime when indicated and appropriate.
• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education.
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
• Promotes and maintains an agency environment that is in compliance with federal, state, and local regulatory agencies.
• Participates in personal and professional growth and development including staff meetings and in-service education.
• Communicates with patients, families, and other health professionals in a manner that conveys respect, caring, and sensitivity.
• Communicates with the physician and/or nurse practitioner regarding patient status and medical intervention.

Qualifications Include:

A current license as an Occupational Therapist in Massachusetts A minimum of one year of recent work experience as an Occupational Therapist is preferred. A graduate of a occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA, as indicated by school transcript or Diploma Has strong clinical skills as well as good oral and written communication Possesses and maintains current CPR Certification Must be a licensed driver with an automobile that is insured in accordance with state and/ or organization requirements and is in good working order, or other reliable means of transportation. Ability to work independently and manage time effectively Possesses strong interpersonal skills, organizational skills and problem solving skills Solid computer skills; prior experience with electronic medical records preferred.

Remote Type: On-site Salary Range: $86,670.93 - $130,006.92
