Job summary

UnitedHealth Group is seeking a Nurse Practitioner for in-home care delivery, traveling to patient homes within the assigned area. This role offers a $10,000 sign-on bonus and a pathway to impactful, patient-centered care without night, weekend, or holiday shifts.

The APC will work with an interdisciplinary team to deliver preventive, ongoing care, coordinate services, and support patients and families in navigating the health system. Strong clinical skills and collaboration are essential.

Qualifications

• Must have ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted license in the practicing state (or ability to obtain by start date).
• Active DEA licensure/prescriptive authority or ability to obtain post-hire.
• Ability to obtain a collaborative practice agreement, if applicable in your state.

Responsibilities

• Perform in-home assessments including medical history review, medication reconciliation, vital signs, and physical exams.
• Manage patients through acute events or chronic condition exacerbations with testing, diagnostics, interventions.
• Support short-term sub-acute rehabilitation needs.
• Ensure accurate documentation including ICD-10 coding.
• Communicate with and collaborate within the interdisciplinary care team.
• Engage in advanced illness and end-of-life discussions.
• Provide counseling and education to patients and caregivers.
