Abode Care Partners in Indiana seeks a compassionate Nurse Practitioner to provide primary care to seniors in patients' homes across New Castle, Greenfield, Muncie, and Richmond. You’ll perform clinical assessments, develop care plans, manage chronic conditions, and collaborate with interdisciplinary teams to improve outcomes.

Qualifications

• Active state NP license.
• Current NP certification (NBN).
• DEA licensure active in state.
• MSN degree in nursing.
• 5+ years nursing experience; 2+ NP experience preferred.
• Strong communication and time management.
• EMR and MS Office familiarity.
• Driver license with valid insurance.

Responsabilités

• Performs evidence-based medical care and plans patient needs.
• Examines and treats chronic and acute illnesses and injuries.
• Orders and interprets labs and diagnostics as needed.
• Collaborates with caregivers and healthcare professionals.
• Provides referrals when necessary.
• Keeps patient records up to date.
