Job summary

UnitedHealth Group in the Kansas City area seeks a qualified Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage and other plan members.

This non-prescribing, field-based role emphasizes education, gap closure, and collaboration with primary care providers to improve health outcomes. Flexible 20–40 hour schedules, with no on-call, weekends, or holidays.

Qualifications

• NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• PAs: Current NCCPA certification and state licensure
• Active, unrestricted licensure in the practicing state without disciplinary actions
• Active and unrestricted driver's license with reliable transportation for home visits
• Ability to navigate varied home environments and perform assessments as needed
• Eligibility process if practicing independently where allowed; apply within specified windows

Responsibilities

• Conduct comprehensive in-home assessments including history, meds, vitals, and exams
• Identify and document diagnoses for care management and planning
• Communicate findings to PCPs to address gaps in care
• Recognize urgent situations and intervene appropriately
• Educate members on diseases, medications, and compliance
• Address social determinants of health and provide referrals as needed
