Job summary

Your Health Organization is seeking a Registered Nurse to serve patients across our Dublin-area service region. As a Nurse Case Manager, you will visit patients at home and in facilities, coordinating with providers and initiating telemedicine as needed.

Travel daily is required to reach patients in homes and facilities, with a standard 8-hour shift, Monday–Friday. The role emphasizes Care Management, documentation, and collaboration with interdisciplinary teams to ensure high-quality, compliant

Qualifications

• Must be a licensed nurse with active RN license in good standing.
• Proven ability to communicate effectively and collaborate with care teams.
• Ability to manage multiple responsibilities and demonstrate sound judgment.
• Basic computer knowledge.
• Ability to travel between offices, facilities, and patient homes.

Responsibilities

• Facilitate receiving medical records from patients’ providers.
• Review medical records and enroll patients in Care Management.
• Initiate a Care Management Plan of Care when eligible.
• Coordinate with care team and providers for patient needs.
• Document Care Management activities and perform on-call duties if required.
