Job summary

Your Health Organization in the Athens area is seeking a Registered Nurse to serve patients as a Visiting Nurse / Nurse Case Manager. The role involves visiting patients at homes and long-term care facilities, coordinating with providers, and initiating telemedicine visits.

You must have reliable transportation as travel is daily. This is a full-time, salary-based position with 8-hour shifts Monday through Friday (8am–5pm).

Qualifications

• Must be a licensed nurse. RN preferred. License in good standing.
• 3+ years clinical experience preferred.
• Experience in community settings preferred.
• Proven ability to communicate and collaborate with care teams, patients, and caregivers.
• Strong written and verbal skills.
• Basic computer knowledge.
• Ability to manage and demonstrate leadership skills.
• Team player with ability to manage multiple responsibilities and sound judgment.
• Must be able to work flexible hours and travel; licensed driver with insured vehicle.

Responsibilities

• Facilitate receiving medical records from patient’s providers and specialists.
• Review medical records.
• Complete consents with patients.
• Enroll patients in Care Management if eligible.
• Initiate a Care Management Plan of Care if eligible.
• Capture diagnoses with specificity and ensure acceptance.
• Complete AWVs for provider review.
• Complete cognitive impairment screenings.
• Complete SDoH assessments and screenings.
• Complete ACPs for provider review.
• Evaluate for home health, hospice, or RPM referrals.
• Evaluate for RPM devices or tools to improve life quality.
• Coordinate and communicate care across teams.
• Reconcile medications and supplements.
• Evaluate for adaptive equipment and DME.
• Assess safe home environment.
• Document Care Management activities accurately.
• On call may be required.
