Job summary

Your Health Organization in Athens area seeks a Registered Nurse to serve as Visiting Nurse (Nurse Case Manager). You will travel to patients’ homes and facilities to evaluate needs, initiate telemedicine, and coordinate care within our care team.

We value strong communication, teamwork, and documentation skills, with 3+ years of clinical experience and a valid RN license. This full-time role offers competitive compensation and benefits.

Qualifications

• Must be a licensed nurse and in good standing.
• Three years of clinical experience preferred.
• Experience in community settings preferred.
• Proven ability to communicate and collaborate with interdisciplinary care teams, patients, and caregivers.
• Strong written and verbal skills.
• Basic computer knowledge.
• Ability to manage multiple responsibilities and exercise sound judgment.

Responsibilities

• Facilitate receiving all medical records from the patient’s primary provider and specialists.
• Review medical records.
• Complete consents with patients.
• Enroll patients in Care Management if eligible.
• Initiate a Care Management Plan of Care if eligible.
• Capture diagnoses with highest specificity.
• Complete AWV’s reviewed by the provider.
• Coordinate care and communicate with care team.
• Travel to patient homes and facilities as needed.
