Qualifications

• Certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care is required.
• Active driver's license and reliable transportation for home visits.
• 1+ years of clinical experience in family or geriatric care preferred.

Responsibilities

• Conduct in-home assessments including medical history and physical exams.
• Document diagnoses for care management and treatment planning.
• Educate members on disease processes and medications.

Skills

Active ANCC or AANP certification

Current NCCPA certification

Proficiency with electronic medical records

Excellent communication skills

Education

Active, unrestricted licensure in the state of practice
