Job summary

Optum Home-Based Medical Care is hiring a Nurse Practitioner or Physician Assistant to deliver in-home care for medically complex, high-risk patients. The role aims to reduce hospitalizations, improve outcomes, and manage chronic illness with longitudinal care.

Ideal candidates will have NP/PA credentials and a Washington license, with a focus on autonomy, geriatrics, and durable patient relationships. Travel throughout Yakima County is common.

Qualifications

• Must be graduate of an accredited NP or PA program.
• Active NP or PA license in Washington.
• Certification by AANP, ANCC, or NCCPA.
• DEA license or ability to obtain.

Responsibilities

• Conduct comprehensive geriatric home assessments.
• Manage chronic and complex medical conditions.
• Diagnose and treat urgent needs in the home.
• Order and interpret labs, EKGs, X-rays, and diagnostics.
• Initiate treatment at point of care.
• Close HEDIS gaps and document HCC diagnoses.
• Perform cognitive, functional, social, and safety assessments.
• Collaborate with primary care physicians and specialists.
