Accompany Health is seeking a Health Advocate to join our team of care professionals. You will visit patients in their homes to support virtual clinicians, collect vitals, and help advance care plans.

You will collaborate with physicians, nurses, behavioral health clinicians, and community health workers to improve health outcomes for patients with complex needs. You will build trusted relationships, facilitate virtual visits, conduct in-home assessments, and navigate social needs and insurance

Formación

• Experience in a patient-facing role (e.g., Medical Assistant, Community Health Worker, Health Coach, EMT).
• Comfortable working in patients’ homes and communities independently, including with medically or socially complex populations.
• Strong interpersonal skills—able to quickly build rapport and communicate clearly with patients and clinicians.
• Basic clinical and technology proficiency (e.g., vital sign collection, using mobile apps, supporting video calls).
• Valid unrestricted driver's license and access to an insured vehicle for daily use.

Responsabilidades

• Build trusted relationships with patients and support high-need individuals to stay healthier at home.
• Set up and support virtual visits with clinicians, ensuring smooth technology use.
• Record vitals and document observations during in-home assessments.
• Support behavioral health visits by collecting vitals and assisting screenings.
• Identify social needs and navigate resources and insurance benefits for patients.
• Reinforce education, coach on adherence, track progress toward goals.
• Support patients in setting goals and overcoming barriers to stay healthy.
• Follow up on referrals, scheduling, and tests; manage delivery of home testing kits.
• Deliver supplies and equipment following AH internal procedures.
• Maintain accurate EHR documentation and updates to the care team.
• Accompany patients to clinical visits for preventive screening and care gaps.
• Performs other duties as assigned
