Job summary

Signify Health is seeking an in-home clinician to conduct comprehensive visits, review medical histories, perform physical evaluations, and deliver diagnostic services as ordered by health plans. You will travel locally and to licensed states, with virtual visits when appropriate to ensure access to care.

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.
