Job summary

Upward Health is seeking a Care Specialist to deliver chronic care management predominantly in patients’ homes and communities, with some virtual visits. You will guide enrollment, facilitate virtual appointments, and ensure adherence to care plans to improve outcomes.

Requirements include a high school diploma or GED; experience in chronic care, home visits, and strong communication skills are highly valued. Auto transportation and reliability are preferred.

Qualifications

• Requires high school diploma or GED.
• Experience with chronic care management or working with chronically ill/elderly patients is preferred.
• Ability to travel within assigned territory using own vehicle.

Responsibilities

• Deliver chronic care management in patients' homes and communities (90% in-home, 10% virtual).
• Guide enrollment and coordinate virtual appointments with providers.
• Educate patients and coordinate care to prevent hospitalizations and improve outcomes.
• Build relationships with patients and families to support adherence to care plans.
• Document care activities and communicate effectively with the care team.
