Job summary

Northwell Health is seeking a Community Health Worker liaison to support vulnerable community members by conducting assessments and in-home visits. You will connect clients with social services, maintain a caseload, and coordinate care with the broader team.

In this role you will mentor CHWs, provide health education, collaborate with care teams, and help members develop actionable health management plans to improve overall well-being.

Qualifications

• High School Diploma or equivalent required.
• 1-3 years of relevant experience required.

Responsibilities

• Serves as a mentor for Community Health Workers
• Provides oversight in care of members to improve health outcomes and promote well-being
• Acts a resource for CHW and members
• Provides support and basic health and education about material and general health care and community resources.
• Conducts one-on-one health education, community outreach, regular in-home visits and service coordination.
• Provides various supports, services entitlements and benefits for members.
• Supports members in developing health management care plans.
• Empowers members by promoting effective approaches, problem solving and lifestyle changes.
• Delivers coaching and mentorship on chronic health conditions and self-care.
• Enters client data into software on a daily basis.
• Advocates for members and their families and collaborates with the care team as needed.
