Job summary

Your Health Organization in Georgia seeks a Community Health Worker to serve patients in the Athens area. The CHW visits patients at home and in ALFs, coordinating visits with providers and ensuring access to medications and DME. Daily travel is required and you will work a full-time, M-F 8-hour shift.

We value reliable transportation, strong communication, and teamwork to improve patient care across facilities, clinics, and homes.

Qualifications

• Certified or licensed healthcare professional in good standing (CMA, Paramedic, or RN).
• Phlebotomy experience preferred; willing to train if needed.
• Public health or home health experience preferred.
• Proficient with technology for remote patient monitoring and EMR.

Responsibilities

• Work closely with care teams to coordinate patient care at home or in facilities.
• Deliver medications and DME to patients' homes and ALFs; review instructions.
• Coordinate telehealth visits between patients, pharmacists, and providers.
• Train patients on RPM equipment and provide ongoing support.
• Ensure timely care and adherence to the care plan; travel between sites daily.
