Job summary

The Child Center of NY strengthens children and families with skills, opportunities, and emotional support to build healthy, successful lives. The Resource Coordinator (Care Manager) is responsible for outreach, enrollment, and guiding individuals and families to appropriate services within an interdisciplinary team.

Home visits are required in the Queens or Long Island area, and the role emphasizes coordinating medical and behavioral health needs while ensuring access to available support

Responsibilities

• Complete intake, enrollment, and monitoring ongoing assessments.
• Facilitate interdisciplinary collaboration among providers, the enrollee, family, caregivers and all available supports.
• Ensure families are aware of all support services and entitlements; provide recommendations and referrals to relevant community resources.
• Advocate (as necessary) for individuals/families by serving as a liaison between families and other programs.
• Conduct office/home/community visits to assess family needs and service delivery.
• Perform outreach to program candidates among client populations within the community.
• Assist in coordination of services for families with medical and mental disabilities.
• Monitor and maintain compliance with Federal, State and Local regulations.
• Monitor compliance using data management systems.
