Mosaic in Arizona invites you to become a Home Provider. You’ll open your home to an individual with an intellectual disability, welcoming them to live with you full-time as part of your family and daily life.

Qualifications

• Must be at least 21 years of age.
• Arizona resident due to state funding restrictions.
• Own/rent your home, apartment, condo, or townhouse; active homeowner's or renter's insurance.
• Reliable transportation, valid driver's license, and current vehicle insurance.
• Internet access and basic computer skills.
• Ability to complete required screenings, background checks, and training.

Responsibilities

• Provide daily living support including cooking, budgeting, cleaning, and decision-making.
• Assist with medical appointments and medication administration.
• Help someone build connections, participate in community activities, and experience a sense of belonging.
• Provide transportation and support with daily activities as needed.
• Maintain documentation and communication with the Mosaic support team.
• Help someone build confidence, develop independence, and pursue personal goals.

Position Snapshot

• Location: Must reside in Glendale, Peoria, or Phoenix, AZ (Must own or rent your current home)
• Supporting: You are welcoming an individual with an intellectual disability to move into your home and live with you full-time as a part of your household and family dynamic
• Average Annual Income: Approximately $42,500 (federally tax-free)
