Job summary

Mosaic is seeking Home Providers in Western Nebraska to open their home and welcome an individual with an intellectual disability to live full-time as part of your household.

You will support daily living, transportation, appointments, and community participation, with training and ongoing team support. This is a work-from-home opportunity that helps people belong while you earn an income and grow through a meaningful role.

Qualifications

• Must be at least 19 years old.
• Must be a current resident of Nebraska (state funding restrictions).
• Own or rent your own home (house/apartment/condo).
• Have active homeowner's or renter's insurance.
• Reliable transportation with valid driver’s license and current vehicle insurance.
• Internet access and basic computer skills.
• Ability to complete required screenings, background checks, and training.

Responsibilities

• Support daily living skills (cooking, budgeting, cleaning, decision-making).
• Assist with medication administration and medical appointments.
• Help individuals build connections and participate in their community.
• Provide transportation and support daily activities as needed.
• Maintain documentation and communicate with Mosaic support team.
• Help individuals build confidence and pursue personal goals.

Position Snapshot

• Location: Must reside in the following Western Nebraska counties: Banner, Box Butte, Cherry, Cheyenne, Dawes, Deuel, Garden, Keith, Kimball, Morrill, Scotts Bluff, or Sioux (Must own or rent your current home)
• Supporting: You are welcoming an individual with an intellectual disability to move into your home and live with you full-time as a part of your household and family dynamic
• Average Annual Income: Approximately $71,800 (federally tax-free) + Monthly Rent & Household Expenses Contribution

Requirements
