Soleo Health is seeking a Therapy Care Pharmacist in Hoover, AL to support the Acute Therapy Care Management Team. This on-site role requires parenteral certification and Pharm D or R.Ph with clinical experience.

You will collect data, collaborate with prescribers, dispense medications, and supervise therapy care staff to ensure high-quality patient outcomes. Join Soleo Health to deliver complex specialty pharmacy services, maintain compliance, and contribute to a patient-centered care model

Qualifications

• Pharm D or R.Ph with clinical experience.
• Active state licensure in the applicable state.
• Experience in home infusion or specialty pharmacy preferred.
• Post-graduate residency or training preferred.
• CPR+ pharmacy computer system experience preferred.
• Excellent clinical and customer service skills.
• Proficient with Microsoft Excel, Word and Outlook.

Responsibilities

• Collect and assess clinical data for specialty therapies.
• Collaborate with prescribers to optimize treatment plans.
• Dispense medications and supervise therapy care staff.
• Document patient interactions in the designated system.
• Coordinate with local and national pharmacy teams for efficiency.
• Ensure activities comply with state/federal law and policies.
• Assist in orientation and training of new staff.
• Supervise therapy care specialists and support personnel.
