Job summary

InfuCare Rx is seeking a licensed pharmacist to join our home infusion team in Maryland. You will support IV therapy programs, collaborate with physicians, nurses, and insurance providers, and ensure safe and effective drug therapy for home patients.

Responsibilities include medication review, sterile compounding oversight, patient counseling, and documentation in patient records. The role requires strong clinical skills, teamwork, and flexibility to support home infusion services.

Qualifications

• Bachelor of Science in Pharmacy (RPh) or Doctor of Pharmacy (PharmD) from an accredited college.
• Currently Licensed as a registered pharmacist in the state of employment.
• Minimum of two years recent experience in an acute-care setting or one-year recent experience in home infusion therapy.
• Experience and strong knowledge base in intravenous admixture services and intravenous therapy.
• Capable of evaluating and monitoring drug therapy in collaboration with physicians and nurses.

Responsibilities

• Coordinates and facilitates new referral/admissions through collaboration with nursing, customer service, and distribution.
• Receives, reviews, and verifies all new patient orders and documents are the same on the order form.
• Assists physician and other healthcare professionals in planning and implementing patient care in the home.
• Monitors outcomes from drug therapy by evaluating laboratory and clinical data.
• Communicates pharmacotherapy results to the care team and documents in patient records.
• Performs sterile compounding and oversees dispensing of pharmaceutical supplies.
• Participates in on-call rotation if applicable.
