Job summary
Option Care Health is seeking a Pharmacist to determine patient suitability for home care and to develop pharmacist-led care plans. You will monitor and adjust therapy, educate patients and staff, and ensure safe, compliant medication delivery in the home setting.
The role requires a Pharmacy degree, active license, and experience in IV or non-sterile compounding or MTM. Travel up to 5% may be required, with competitive benefits and a strong compliance culture.
Qualifications
- Graduate of an accredited School of Pharmacy.
- Current, active Pharmacy license as granted by the State Board of Pharmacy.
- One of the following: (a) sterile IV admixture experience, (b) non-sterile compounding/immunizations/MTM, (c) ASHP residency, (d) APPE with school, or (e) equivalent experience.
Responsibilities
- Determines suitability of patients for home care and collects necessary information.
- Designs and monitors patient-specific pharmacy care plans for home care patients.
- Modifies care plans based on monitoring data and lab values.
- Provides medication-use education to staff, patients and caregivers.
- Ensures continuity of pharmaceutical care between home and other settings.
- Maintains equipment and provides training on its use and maintenance.
