Job summary

Amerita, Inc. is seeking a Pharmacist to join our home infusion Pharmacy team. The Pharmacist will report to the Pharmacy Manager and work in our City, State branch, performing pharmaceutical care activities for customers and patients within the service area.

You will collaborate with physicians, nurses and other healthcare professionals to monitor drug therapy, ensure compliance with USP standards, and support timely patient care. Strong communication and documentation skills are essential.

Qualifications

  • Pharmacist licensure in state of practice is required.
  • BS in Pharmacy is required; Doctor of Pharmacy is preferred.
  • Minimum of two years of recent experience in an acute‑care setting or one year in home infusion therapy.
  • Experience with compounding infused drugs.
  • Excellent verbal and written communication skills.
  • Flexible schedule with on‑call evenings and weekends as scheduled.
  • 0-25% travel may be required.

Responsibilities

  • Evaluates and monitors drug therapy for home care patients with physicians, nurses and other healthcare professionals.
  • Adheres to organization policies and applicable state and federal pharmacy laws.
  • Participates in quality assurance activities and accreditation standards.
  • Coordinates and facilitates referrals and admissions with nurses, customer service and distribution.
  • Reviews and verifies new patient orders and documents on the order form.
  • Assists physicians with home care planning and therapeutic objective setting.
  • Documents and monitors patient drug information and outcomes through labs and records.
  • Provides drug information to patients and healthcare professionals and maintains thorough documentation.
  • Oversees pharmacy technician functions as required by Pharmacy Board rules.

Job Locations: US-MA-HUDSON, US-RI-PAWTUCKET, US-RI-PROVIDENCE, US-RI-RIVERSIDE.

Salary Range: USD $65.00 - $70.00 per hour.

Position Type: Full-Time.

Responsibilities
  • Evaluates and monitor drug therapy relative to the total needs of the home care patient in collaboration with physicians, nurses and other healthcare professionals.
  • Adheres to organization policies and procedures and all applicable state and federal laws and regulations governing pharmacy practice.
  • Adheres to all appropriate standards of accreditation and participate in quality assurance activities.
  • Coordinates and facilitates new referrals and admissions through collaboration with nurses, customer service and distribution.
  • Reviews, receives and verifies all new patient orders and documents on the respective order form.
  • Assists the physician and other involved healthcare professionals with:
    • Planning and implementing patient care in the home;
    • Setting therapeutic objectives for drug therapy;
    • Establishing parameters for monitoring the safety and efficacy of drug therapy;
    • Preparing regular progress reports.
  • Conducts and documents initial and ongoing assessment of specific patient, drug and disease information to identify real and potential drug-related problems or needs.
  • Monitors the outcomes of drug therapy by collecting, evaluating and interpreting the results of laboratory data, clinical data and other pertinent patient information.
  • Provides consultation and answers inquiries from patients, families, physicians and other healthcare professionals regarding drugs and their use.
  • Maintains thorough and appropriate documentation in the customer/patient record of all clinical activities and communication with patients, physicians and other healthcare professionals.
  • Processes and checks all medications, supplies and equipment prior to dispensing from the pharmacy.
  • Compounds sterile mixtures and non-mixtures, including, but not limited to parenteral nutrition, antibiotics, chemotherapy, hydration and narcotic analgesics, using aseptic technique where state laws permit.
  • Ensures compounding/clean room areas and related equipment are cleaned, sanitized and maintained in compliance with company policies and procedures, and USP 797 and USP 800 standards.
  • Documents all activities on appropriate logs.
  • Pharmacists' duties are extended to include overseeing the Pharmacy technician functions as required by Pharmacy Board rules.
Qualifications
  • Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred.
  • Registered Pharmacist License in state(s) of practice.
  • Minimum of two (2) years of recent experience in an acute‑care setting or one (1) year of recent experience in home infusion therapy.
  • Experience with compounding infused drugs.
  • Proven record of evaluating and monitoring drug therapies for home care patients.
  • Good organizational and time‑management skills.
  • Participation and membership in professional societies and organizations is desirable.
  • Excellent verbal and written communication skills.
  • Flexible schedule with the ability to work on‑call evenings and weekends on an as‑scheduled basis.
  • Percentage of Travel: 0-25% – Minimal travel may be required.

Physical Requirements: Constant sitting and typing on a keyboard with fingers, and occasionally standing and walking. The role requires the ability to push/pull and lift/carry 10-30 lbs.
