Job summary

Patient Care Infusion is seeking a Home Infusion Pharmacist in Tempe, AZ to take ownership of clinical practice in a home-based setting. The role emphasizes collaboration with physicians and nurses to optimize patient outcomes, with no retail duties and strong emphasis on patient-centered care.

The position requires a Pharmacist license in Arizona, at least 2 years of experience, and offers a $145,000–$160,000 salary plus a $7,500 sign-on bonus.

Qualifications

PharmD or B.S. in Pharmacy required.
Active, unrestricted Arizona Pharmacist license required.
2+ years of pharmacist experience required.

Responsibilities

Perform pharmacy tasks including calculations, data entry, and workflow management.
Oversee and verify sterile compounding by technicians under USP 797/800.
Review and verify medication orders for accuracy, interactions, contraindications, and dosing.
Monitor drug therapy, interpret labs, and adjust care plans to optimize outcomes.
Coordinate patient care with physicians, nurses, and the care team.
Educate patients and nurses on therapy expectations.
