Job summary

Option Care Health is seeking a per diem Home Infusion Nurse II to provide intermediate nursing care in patients' homes or infusion suites. The role requires an active RN license, CPR certification, and at least 1 year of nursing experience.

The work supports continuity of care under a physician’s plan and involves coordinating with the clinical team. You will train and mentor junior nurses, document care in real time, and participate in on-call duties and discharge planning.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 year of nursing experience.
• Current CPR certification required.

Responsibilities

• Initiates and provides intermediate direct patient care in the home or infusion suite.
• Administers medications and educates patients and caregivers.
• Documents care in real time and communicates with the clinical team.
• Coordinates discharge planning and prepares summaries for transfers.
• Mentors Infusion Nurse I’s and participates in training activities.
