Job summary

Option Care Health is seeking an Infusion Nurse II to coordinate and provide intermediate home-based nursing care under a physician's plan of care. The role requires delivering direct patient care, organizing treatments, and educating patients and families for independence.

Responsibilities include clinical documentation, collaboration with the clinical team, and discharge planning. RN licensure and CPR are required, with 1+ year nursing experience preferred.

Qualifications

• Active and unrestricted RN license in state of practice.
• Minimum of 1 year nursing experience.
• Current CPR certification required.

Responsibilities

• Initiates and coordinates intermediate nursing plan of care treatments.
• Assesses patient needs and status during visits and updates physician orders.
• Provides training and mentorship to Infusion Nurse I’s as needed.
• Educates patients and families to achieve independence with prescribed therapy.
• Communicates with clinical team, patients, and families to ensure continuity of care.
• Prepares and submits clinical documentation in real time for timely billing.
• Coordinates discharge planning and provides discharge summaries.
