Job summary

Option Care Health, Inc. seeks an Infusion Nurse II to coordinate and provide intermediate direct patient care in the home or an infusion suite, ensuring patient safety under a physician’s plan of care and under structured supervision.

You must hold an active RN license, have at least 1 year of nursing experience, and CPR certification. The role includes real-time clinical documentation, teaching and mentoring staff, discharge planning, and potential travel up to 100%.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 year of nursing experience.
• Current CPR certification required.

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care treatments, evaluating patient progress towards goals.
• Assesses patient needs and physical status at each skilled visit and informs physician of significant changes.
• Provides training and mentorship to Infusion Nurse I’s as needed.
• Provides effective teaching to patients and families for independence with prescribed therapy and care needs.
• Maintains real-time clinical documentation and coordinates discharge planning.
