Job summary

Option Care Health, Inc. is seeking a registered nurse to provide nursing services for on-site referral coordination, patient assessment, education and training for home IV therapy in South Carolina.

You will coordinate with pharmacists, IP team and discharge planners to initiate home IV therapy, monitor progress, and ensure safe care. Requires SC RN license and 2+ years in home IV therapies; valid driver's license preferred.

Qualifications

• Current SC RN license is required.
• 2+ years experience in home IV therapies preferred.
• Excellent clinical, communication and organizational skills.

Responsibilities

• Performs initial patient assessment to determine suitability for home IV therapy.
• Educates patient or caregiver on home IV therapy and device operation.
• Coordinates initiation with Pharmacy, IP team and discharge planners.
• Monitors patient progress and documents care plans and outcomes.
