Job summary

Option Care Health, Inc. is seeking an Infusion Nurse II to coordinate and provide intermediate in-home patient care under a physician's plan of care.

You will work under structured supervision, hold an active RN license, and bring at least 1 year of nursing experience with CPR certification to ensure patient safety. Responsibilities include administering treatments, educating patients and families, documenting care in real time, and collaborating with a multidisciplinary team.

Qualifications

• Active RN license in the state of practice.
• Minimum of 1 year nursing experience.
• Current CPR certification required.
• Infusion nursing experience preferred.

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care treatments and educates patients and families.
• Assesses patient needs and re-evaluates progress during visits with physician orders.
• Provides training and mentorship to Infusion Nurse I’s to ensure safety and compliance.
• Documents care in real-time and coordinates with clinical teams for continuity of care.
• Performs discharge planning and reports patient progress and goals to physicians.
