Job summary

InfuCareRx, Inc. in Pottsville, PA is seeking a Nursing Case Manager/Specialty to lead the clinical team in an infusion pharmacy setting. You will manage referrals, schedule RN home visits, and coordinate ongoing patient care with pharmacists, intake staff and dietitians.

Qualifications

• Must hold current RN license in good standing.
• Bachelor's degree; CRNI preferred.
• BLS/CPR certification is required.
• Minimum 3 years of home infusion nursing experience.
• 1-2 years supervisory experience in home care preferred.
• Working knowledge of insurance requirements.
• Practical knowledge of ACHC regulations as it applies to infusion home care.
• Strong organizational skills and written/verbal communication skills.

Responsibilities

• Lead nursing staff within the interdisciplinary clinical patient care team.
• Manage referrals for the Nursing Department and RN home visit scheduling.
• Coordinate patient care with infusion office pharmacists, Intake team members and Dietitians.
• Oversee home infusion care provided to patients with acute and chronic diseases.
• Stay current on new therapies, industry trends, and accreditation standards for home infusion services.
• Other duties as assigned by management.
