Job summary

InfuCare Rx, a leading home infusion provider, is seeking a Nursing Case Manager to lead the clinical patient care team within our infusion pharmacy office setting. You will manage referrals, schedule RN home visits, and oversee ongoing patient care in collaboration with pharmacists, intake and dietitians.

The role requires an active RN license, BLS/CPR, and a minimum of 3 years in home infusion nursing, with supervisory experience preferred.

Qualifications

• Current registered nurse license in good standing.
• Bachelor’s degree; CRNI preferred.
• Current BLS/CPR certification is required.
• Minimum 3 years home infusion nursing experience.
• 1–2 years supervisory experience in a home care setting preferred.
• Knowledge of insurance industry requirements preferred.
• Practical knowledge of State and Federal regulatory requirements and ACHC regulations as applied to infusion home care.

Responsibilities

• Coordinates patient discharges to home infusion services with hospital case managers, prescribers, home health agencies and other referral sources.
• Oversees home infusion care for patients with acute and chronic diseases.
• Utilizes knowledge of IV access devices, patient assessments, multidisciplinary care planning and patient education to optimize care.
• Fosters relationships with healthcare professionals for educational purposes and partnerships.
• Develops nursing care plans in collaboration with prescribers and care teams to ensure optimal outcomes.
• Keeps current on therapies, industry trends and accreditation standards for home infusion services.
• Performs other duties as assigned by management.

Job Summary

The Nursing Case Manager functions as the lead nursing staff member of the interdisciplinary clinical patient care team, within an infusion pharmacy office setting. Responsible for referral management for Nursing Department, RN home visit scheduling, and ongoing patient care management in collaboration with infusion office pharmacists, Intake team members and Dietitians.

Essential Job Responsibilities

• Supports hospital case managers, prescribers, home health agencies, LTC facilities, liaisons, and other referral sources to coordinate patient discharges to home infusion services.
• Oversees home infusion care provided to patients with acute and chronic diseases.
• Knowledge of IV access devices, patient assessments, multidisciplinary care planning, patient education, IV medication administration, laboratory values and ongoing clinical patient monitoring.
• Fosters new and existing relationships and maintains contact with other industry healthcare professionals for educational purposes and potential partnerships.
• Identifies patient and caregiver home infusion care appropriateness, patient response to care and helps to develop nursing care plans in partnership with prescribers, RN field staff and other members of the patient’s healthcare team to ensure optimal patient outcomes.
• Remains current on new therapies, industry trends, and accreditation standards for home infusion services.
• Other duties as assigned by management.

Position Qualifications
