Job summary
Option Care Health Inc. is seeking an entry-level Clinical Pharmacist to provide safe and appropriate pharmacy services for home care patients.
The role collaborates with Nursing, monitors goals, and educates patients and caregivers on medication use while ensuring compliance with regulatory standards. The position emphasizes clinical pharmacy in an alternate setting, with responsibilities spanning patient assessment, pharmacotherapy management, education, and equipment oversight within a home
Qualifications
- Graduate of an accredited School of Pharmacy.
- Current, active Pharmacy license as granted by the State Board of Pharmacy.
- One of the following: sterile IV admixture, non-sterile compounding, MTM, residency, or equivalent experience.
Responsibilities
- Assess and determine patient suitability for home care.
- Design and monitor patient-specific pharmacotherapy plans.
- Educate patients, caregivers and staff on medication use.
- Ensure continuity of pharmaceutical care across home and patient-care settings.
- Maintain safety, accuracy and regulatory compliance in dispensing.
