Job summary
Option Care Health is seeking an entry-level Clinical Pharmacist to provide safe, appropriate pharmacy services in home care settings. You will coordinate with Nursing, educate patients and caregivers, and ensure high-quality, compliant medication use across settings.
The role requires a valid pharmacy license, graduation from an accredited school of pharmacy, and possible ASHP residency or equivalent experience.
Qualifications
- Experience providing customer service to internal and external customers.
- Basic Microsoft Word skills for document handling and correspondence.
- Proficiency in Microsoft Excel for data and calculations.
- Proficient Microsoft PowerPoint usage for presentations.
Responsibilities
- Assess patient suitability for home care and collect relevant information.
- Develop and monitor patient-specific pharmacy care plans for home care.
- Modify plans based on monitoring data and lab values.
- Educate patients, caregivers, and staff on medication use.
- Ensure continuity of pharmaceutical care across home and clinical settings.
- Manage and troubleshoot medication administration equipment and inventory.
- Respond to drug information requests with evidence-based answers.
