Affinity Hospice Care is seeking a per diem Registered Nurse to support home health services in California. The role focuses on reviewing OASIS assessments, ensuring accurate ICD-10 coding, and collaborating with clinicians to maintain data integrity for patient care.

Responsibilities include monitoring visit utilization, participating in quality improvement, and staying current with best practices through ongoing education. A valid CA RN license and CPR certification are required.

Qualifications

• Active Californian RN license in good standing.
• 1–2 years of clinical home health experience.
• Working knowledge of OASIS assessments and ICD-10 coding.
• Knowledge of federal regulations and state licensure requirements.
• Strong communication skills with attention to detail.
• Ability to work independently with minimal supervision.
• Proficiency with computer systems and electronic health records.
• Current CPR certification (AHA).
• Valid driver's license with reliable insured vehicle.

Responsabilités

• Review OASIS assessments for completeness, appropriateness, and regulatory compliance.
• Ensure accurate ICD-10 coding and sequencing.
• Collaborate with clinical staff to resolve documentation issues.
• Monitor visit utilization and flag financial risks.
• Identify trends affecting agency outcomes and process measures.
• Conduct OASIS inter-rater reliability visits.
• Participate in Quality Improvement and Corporate Compliance activities.
• Stay current with best practices through ongoing education.
• Perform other duties as required for patient services.
