Job summary

Hospice of the East Bay is seeking a Registered Nurse (RN) Case Manager in Pleasant Hill, CA. The role focuses on individualized care planning, coordination with the interdisciplinary team, and holistic, family-centered hospice services.

The RN Case Manager assesses patient and family needs, implements care plans, educates caregivers, and ensures compliance with regulatory standards while supporting a compassionate, patient-centered approach.

Qualifications

• Current California Registered Nurse License required.
• Minimum of one year of experience working as a professional nurse in an acute clinical setting required.
• CPR certification required.
• Must have current California Driver’s License and a car in good working condition.
• Prior hospice/home care experience preferred.
• Hospice and Palliative Care Nurse Certification highly desirable.
• Must possess basic computer skills.

Responsibilities

• Identify and address signs of actively dying.
• Communicate and collaborate with patient care, MSW and SCC.
• Assess all aspects of the patient’s pain and develop an individualized pain management plan.
• Coordinate and manage patient care with awareness of insurance benefits and authorizations.
• Educate patients and families using teach-back methods.
• Document nursing assessments, goals, care provided and outcomes.
• Coordinate and present patient information at Interdisciplinary Team Meetings.
• Ensure compliance with state and federal regulations and accreditation standards.
