Qualifications

• Minnesota RN license required or eligible.
• 2+ years hospice experience preferred or related complex care.
• Excellent assessment and clinical decision-making skills.
• Strong communication and relationship-building abilities.
• Reliable transportation and willingness to travel across territory.

Responsibilities

• Serve as the primary Hospice RN Case Manager for assigned patients and families.
• Complete comprehensive nursing assessments and develop individualized care plans.
• Manage pain and symptoms while promoting comfort and quality of life.
• Educate patients, caregivers, and families throughout the hospice journey.
• Coordinate care with physicians, social workers, chaplains, and therapists.
• Maintain accurate EMR documentation and ensure Medicare hospice compliance.
